Sports Physicals

Did you know the student athlete in your family has to have a qualifying sports physical every three years to be eligible to participate in sports? And, well-child exams are recommended yearly.

Summer is a great time to schedule exams your child may need, and MLHS can help you out with that.

A “coupon” letter can be found on the mlhs.org website under the red Patient Portal button (Forms for Download- Sports Physical Forms.) (Click here to download the Sports Form PDF). If you bring in this letter, your out-of-pocket expense, whether it be self-pay, or what’s left after your insurance is billed, will only be $60. Some insurance companies will pay for the entire visit. This is a substantial savings for families with student athletes.

Included in the exam are tests important for your child to have. These include: a physical/well-child exam, hematocrit (hemoglobin and anemia check for females), vision screening, hearing screening, and immunizations.

You can call the main number (320-532-3154) to schedule your appointment at any clinic, with any provider you choose. Remember that on Mondays, the Onamia clinic is open until 8 p.m. to accommodate working parents’ schedules.
