Did you know the student athlete in your family has to have a qualifying sports physical every three years to be eligible to participate in sports? Summer is a great time to schedule exams your child may need, and Mille Lacs Health System can help you out with that.
Along with the state-mandated Qualifying Physical Clearance forms needed for your appointment, a “coupon” letter can be downloaded on the mlhs.org website under the red Patient Portal button drop-down menu (Forms for Download- Sports Physical Forms.) If you bring in that letter, your out-of-pocket expense, whether it be self-pay, or what’s left after your insurance is billed, will only be $60. Some insurance companies will pay for the entire visit. This is a substantial savings for families with student athletes. Make sure to fill out the patient portions of the other four pages of required information (medical provider will fill out their portion at the appointment) and bring these to the appointment with your child.
Included in the exam are tests important for your child to have. These include: a physical/well-child exam, vision screening, hearing screening, and immunizations.
This summer, Sports Physical Exams will be held: In Onamia, Tues. July 25, from 1 to 5; and Thurs. Aug 10, 9-noon. In Isle, Thurs. July 27, from 9 to noon; and Tues., Aug 15, 1-5.
